Response:	No, Medical Release forms do not need to be notarized for MYSA league play. Some out-of-state or international tournaments may require this.

Response:	MYSA forms, minutes and other files are in a Portable Document Format (pdf) so that they cannot be altered and are quick to update. To view and print these files, your computer must have the Adobe Acrobat Reader installed. Reader software is free to download from Adobe's website at: http://www.adobe.com/products/acrobat/readstep2.html
