Online Domestic Travel Permit Instructions

The travel form must be used anytime your team is traveling outside the state of Minnesota (exception: traveling extra-territorial/Wisconsin affiliate members). Please fill in the applicable information as completely as possible. All asterisked fields are mandatory.

Players listed on the official roster at the time of travel and any guest players listed on the online application will be considered the traveling players to the tournament. Those players will be covered by Minnesota Youth Soccer Association medical insurance during time of travel.

Payments may be handled online by using a credit card or by mailing a check to MYSA with the proper mail-in form. The form(s) must be completed in its entirety. Upon completion of the form(s) you will be notified by mail with the appropriate approved documents enclosed.

In the case where a team is traveling before member passes and official rosters have been printed and distributed, please contact your club registrar. Registrars may contact Ranae Orr at 952-933-2384 (800-366-6972) or ranaeorr@mnyouthsoccer.org in order to grant permission to release the passes to another specified club member.

Paper Form Domestic Travel Permit Instructions

Please fill in the applicable information as completely as possible. On the line labeled League/Home Association, please enter your team code, which includes the 3-letter club designation, age, gender, and level of play (e.g. BLM15GC1).

Return the form to the MYSA office along with your check (fees listed above). If a player is a guest player, indicate that and note the team the player is registered to. You may list two tournaments on one (paper form only) travel permit. Write the second tournament information in the bottom section of the permit titled Travel to participate in GAMES by using the same information wording as the section above for the first tournament.

Foreign Travel Permit Instructions
  1. Submit an online MYSA Travel Permit request and appropriate fees
    (separate payments to MYSA and U.S. Soccer). See fee schedule.
  2. Complete all U.S. Soccer forms:

Documents Required for USSF Approval:

  • U.S. Soccer Application for Foreign Travel.
  • Signed Ted Stevens Amateur Sports Act for Foreign Travel.
  • Copy of Official brochure, pamphlet, invitation or other applicable material of the tournament or games Host.
  • Copy of Official roster (printed and sent in by MYSA).
  • Check for the non-refundable Travel Application Fee of $50 made payable to U.S. Soccer and appropriate MYSA fee.

Submit completed forms to the MYSA office for their review and state certification of U.S. Soccer forms at least thirty (30) days prior to the date of departure. (Applications not received 30 days prior to departure may be denied by U.S. Soccer.) MYSA will forward all documentation to U.S. Soccer for review and approval. Approved applications will be sent out by U.S. Soccer.
