ANNUAL CLUB AFFILIATION
In order to renew an Affiliate or Extraterritorial Membership in MYSA, an existing member in good standing with MYSA must complete the Annual Club Affiliation process by September 1 of each year. Clubs are encouraged to complete this sooner than September 1, as affiliation is required before a club can register players or teams to the corresponding season.
ANNUAL COMPLIANCE CHECKLIST
A. Updated club contact information of current officers and administrators.
B. Proof of any of the following:
- Current registration of their corporation with the appropriate state agency.
- Minnesota: link to file annual corporate renewal with the Minnesota Secretary of State office [https://mblsportal.sos.state.mn.us/Business/Search]
- Wisconsin: link to file annual report with Wisconsin Department of Financial Institutions
- Current registration of their parent organization with the appropriate agency and an updated certificate of compliance.
- Minnesota: link for determining corporate status of your organization of parent organization to determine status of parent organization [https://mblsportal.sos.state.mn.us/Business/Search]
- Proof of continued government affiliation with updated certificate of compliance.
C. Current copies of Articles of Incorporation and Bylaws
D. Current club development plan
E. Annual Club Affiliation fee of $350
